  • Family and significant relationships

  • Details of presenting issue(s)

  • Medical details, including conditions and medication relevant to the use of hypnotherapy

  • Session summary - a brief record of the content of the session

Directly from you either before or during hypnotherapy sessions via

  1. Why I have this personal information

To offer you a safe and ethical hypnotherapy service, I need to hold certain personal information from the point of our initial contact, until after our hypnotherapy sessions have ended. Your security is important to me, and I will take every precaution to keep all information shared with me safe and secure and only use it for the purpose for which it was given to me.

 

  1. Sharing information

I do not pass your information to third parties unless I have your permission to do so. With your permission I may also use your information on my website or other marketing information to give examples of my services.

 

  1. How I store your personal information

 

Paper

  • Therapy agreement – including GDPR notice

  • Brief notes from each session

  • Copies of additional documents such as GP letters or other papers related to the conduct of hypnotherapy sessions

Your electronic information is securely stored on Microsoft Office, Google Mail using password protected access. Paper documents related to our hypnotherapy sessions are held in a locked cabinet.

 

  1. How long do I keep your information?

I hold electronic and paper documents relating to your hypnotherapy sessions for eight years from the end of your sessions to meet my insurance and legal obligations.  After that period I will delete or securely destroy the information.

 

 

 

  1. Website and cookie information

​My website is hosted on the Wix.com platform. Wix.com provides me with the online platform that allows me to tell you about my services. Your data may be stored through Wix.com’s data storage, databases and the general Wix.com applications. They store your data on secure servers behind a firewall.

 

When you visit my Website I may collect certain information such as browser type, operating system, etc. This information is used to analyse how people use my site, such that I can improve my service.

 

​I use cookies on my Website. Cookies are very small files which a Website uses to identify you when you come back to the site and to store details about your use of the site. Cookies are not malicious programs that access or damage your computer. I use cookies to improve the experience of people using my Website.

You can access the personal information I hold on you by contacting me (see 1. ‘My contact details’). I will provide a copy of the information free of charge.

 

  1. Lawful basis for processing your personal information

GDPR states that there must be a lawful basis for processing personal data. I collect this data initially to allow me to fulfil a contract between myself as a Clinical Hypnotherapist involved in the provision of a healthcare service to you, the client.

 

  1. Your data protection rights

Under data protection law, you have rights including

  • The right to ask me for a copy of your personal information.

  • The right to ask me to correct personal information you think is inaccurate. You also have the right to ask me to complete information you think is incomplete.

  • The right to ask me to erase your personal information. I can decline to do this if I need this information to fulfil my contractual, insurance and legal obligations.

  • The right to ask me to restrict the processing of your personal information in certain circumstances. I can decline to do this if I need this information to fulfil my contractual, insurance and legal obligations.

 

There is no charge for exercising these rights. If you make a request, I have one month to respond to you.

You can also complain to the ICO if you are unhappy with how I have used your data.
